Liability Waiver

Nurses and Nurse Practitioners must complete a free liability waiver and submit it to the Oregon Nursing Board. You are responsible for completing and submitting this form annually - you will not receive a reminder.

Physicians and other Providers must complete a free liability waiver and submit it to the Oregon Medical Board. You are responsible for completing and submitting this form annually - you will not receive a reminder.

Patients are required to complete a waiver of liability prior to receiving any services from Occupy Medical. This form will be provided and explained to you upon request. If you are unable or unwilling to complete this form, alternative services will be recommended to you.
